Web Accessibility Complaint Form
The complaint or grievance will be investigated by Phoenix Academy of Excellence, and/or another party designated by the school district. Upon receipt of your email the investigator will provide effective communication of information you are unable to access due to a barrier. The procedures to be followed are:

An investigation of the complaint shall be completed in a timely manner. -
The investigator shall prepare a written report of the findings and conclusions. -
The investigator shall contact the Complainant upon conclusion of the investigation to discuss the findings and conclusions and actions to be taken as a result of the investigation.
Note: If the student is transferring from a school outside the county, state, country, or a private institution, please attach the most recent report card and standardized test scores for scheduling purposes.
